Donation Request Form Guidelines:

1. Donation requests will only be considered if they are in the state of Minnesota or Fargo-Moorhead area, and benefiting the state of Minnesota. Out-of-state requests will be reviewed on a case-by-case basis, and under normal circumstances, out-of-state requests will NOT be honored.
2. Due to the number of requests received, Minnesota State University Moorhead Athletics will only honor one donation request per requesting agency/individual each academic year.
3. Return postage is the sole responsibility of the person/organization requesting the donation if your request is honored and items are sent to you in the mail.
4. If your event has a flier or brochure, please submit that with your request.
5. The Donation Request Form must be completed in its entirety, printed off, then signed by the requestor. All forms must then be sent to:

*Signed forms can also be faxed to 218-477-2324, Attn: Jason Sobolik
 
Filling out and sending in the Donation Request Form for does NOT guarantee your request will be filled, all questions can be directed to one of the above two individuals. Please allow at least two (2) weeks for all Donation Request responses. MSUM Athletics will do its best to promptly respond to all requests within two (2] weeks upon receipt of the request and appreciates your patience and understanding in this matter.
